Box 1-D. Carbon monoxide incident. 140 7th Avenue Extension, Snyder Twp.

In route: 09:04 (8min), on scene: 09:06 (2min), available: 09:52.

Brockway ambulance was called to the residence for a person having difficulty breathing.  Upon arrival their CO detectors were going off. They requested fire to investigate.  Crew investigated and found very low levels of CO in the residence.

Units: Rescue 1.  Chief 1 had the command.

Box 1-A.  Carbon monoxide alarm.  1819 Main Street, Brockway Borough.

In route: 20:49 (7min), on scene: 20:51 (2min), Available: 21:02.

A reported CO alarm going off in a residence. Truck crew investigated and found no CO. It was determined to be a faulty detector.

Box 1-A-HH.  Outside fire.  1306 Main Street, Brockway Borough.

In route: 14:50 (5min), on scene: 14:51 (1min), available: 14:59.

Reported as a tire on fire outside the building of Brockway Automotive.  Engine 1 crew investigated and determined the tire caught fire from being leaned up against the chimney.  Crew extinguished the smoldering tire, checked for extension, and went in service.

Units: Engine 1, Rescue 1.  Deputy 1 had the command.

Box 1-D. Vehicle crash.  4189 Route 219, Snyder Twp.

In route: 13:40 (6min), on scene: 13:44 (4min), available: 14:28.

A single vehicle accident. Vehicle into a tree. Rescue 1 crew assisted with hazard control.

Box 1-C. EMS assist. 42 Thompson Drive, Snyder Twp.

Requested for an EMS assist. Brush 1 responded with 4 to assist.

Box 1-B.  Traffic control.  15706 Route 28, Snyder Twp.

Dispatched for a truck stuck on the hill blocking traffic.  Before going in route station was advised that the ambulance was reporting the truck had gotten moving and there was no obstruction.  Chief 1 placed the box in service.

Box 1-C.  Vehicle crash.  Rattlesnake Road at Siple Road, Snyder Twp.

In route: 02:56 (8min), on scene: 03:04 (8min), available: 03:40.

Reported as a single vehicle crash into a pole.  Rescue arrived to find a vehicle off the roadway into a pole. One occupant had fled on foot, the other was in EMS care.  Crews performed hazard control and traffic control until the wrecker company removed the vehicle.

Units: Rescue 1, Engine 1.  Chief 1 had the command.

Box 1-C.  Vehicle crash w/entrapment.  Route 219 at Broad St. Ext., Snyder Twp.

In route: 14:42 (8min), on scene: 14:45 (3min), Available: 16:17.

Reported as a 2 vehicle crash, a tractor trailer and a passenger car, with entrapment.  Chief 1 arrived reporting entrapment of the driver in the passenger car.  Rescue crew set up stabilization and began extrication.  Extrication complete at 15:01 (16min).  Engine 4 was requested to set up the landing zone.  Pumper 15 responded to the scene for manpower.  Elk Engine 3 was called in for cover at station 1.

Elk Box 701.  Vehicle crash.  Route 153, Horton Twp.

In route: 10:24 (7min), Available: 10:36.

Dispatched to assist Elk 7 with a crash.  Reported as a one vehicle crash.  Units were unable to find any crash and went available.

Box 4-UN.  Vehicle crash.  Route 219 in the area of Brubaker Road, Washington Twp.

In route: 08:10 (7min), on scene: 08:16 (6min), available: 09:03.

Dispatched auto aid with station 4 for a three vehicle crash with injuries.  Brockway ambulance reported on scene with 3 vehicle blocking the roadway, no entrapment.  Crews performed hazard control.

Units: Rescue 1, Engine Rescue 4.

Box 15-C.  Flue Fire.  1924 Egypt Road, Warsaw Twp.

Dispatched auto-aid for a flue fire. Deputy 15 arrived on scene and held it to the units responding. Units had not made it out of the station yet.

Box 1-A.  Vehicle crash.  Main Street at Cherry Street, Brockway Borough.

In route: 15:40 (3min), on scene: 15:41 (1min), Available: 16:08.

Crews were at the station cleaning up equipment from the dwelling fire as they were dispatched.  Reported as a two vehicle crash, head-on with fluid leaking.  Crews performed hazard control.

Units: Rescue 1, Engine 1, Brush 1.  Chief 1 had the command.


Alarm # 5

1-24-2010  14:06 hrs.

Box 1-B.  1st alarm dwelling fire.  3058 Kearney Road, Snyder Twp.

In route: 14:12 (6min), on scene: 14:14 (2min), Available: 15:19.

Reported as a chimney fire.  Chief 1 arrived reporting a heavy smoke condition in the basement and heavy smoke from the chimney.  Truck crew accessed the building to check for extension.  They found some light extension into the wall and active fire in the attic.  Crews made quick work of containing the fire keeping the damage to a minimum.  Crews used ground ladders to access the chimney.

Units: Truck 1, Engine 1, Engine 4, Engine 15, DuBois Engine 71, Rescue 1, Tanker 1, Tanker 4, Tanker 15, Engine 2-17 RIT.  Chief 1 had the command.

1-18-2010  14:27 hrs.

Box 1-D.  Vehicle crash.  4189 Route 219, Snyder Twp.

In route: 14:32 (5min), on scene: 14:37 (5min), Available: 15:45.

Reported as a two vehicle crash involving a semi-truck and a passenger car.  Crews controlled the hazards and set up traffic control. Route 219 was completely blocked. Chief 1 requested Elk 7 for assistance with traffic at the county line.  Once the vehicle were removed the road was opened and the units returned.

Box 1-A. Vehicle crash. Main Street at 2nd Avenue, Brockway Borough.

In route: 20:22 (6min), On scene: 20:23 (1min), Available: 20:47.

Reported as a two vehicle crash with injuries.  Chief 1 arrived and held it the the rescue. Crews performed hazard control.

Units: Rescue 1.  Chief 1 had the command.


Alarm # 2

1-4-2010  19:22 hrs.

Box 1-B.  1224 Route 28, Snyder Twp.

In route: 19:28 (6min), On scene: 19:35 (7min), Available: 20:02.

Reported as a vehicle rollover with patient still in the vehicle. While in route control advised that this was off the road behind the residence.  Crews arrived and went in search of the vehicle. Following the donut marks through the field in the 18" of snow they came upon a vehicle. The patient was not entrapped. Crews assisted EMS with loading the patient on a backboard and transporting to the ambulance. Another crew controlled the hazards.

Box 1-A.  Vehicle crash.  Broad Street at Record, Brockway Borough.

Reported vehicle into a tree. Chief 1 arrived reporting the occupants had fled. No resources were needed and placed the box in service.

Box 15-E.  Dwelling fire.  57 Circle Drive, Polk Twp.

In Route: 21:56 (8min), available: 22:01.

Dispatched for a dwelling fire. Station 15 officer arrived reporting a controlled burn and placed the box in service.

12-30-2009  13:19 hrs.

Elk Box 701.  Vehicle crash with entrapment.  Route 219 at McGlaghlin Road, Horton Twp.

In route: 13:25 (6min), on scene: 13:29 (4min), Available: 14:31.

Elk requested rescue 1 to the scene for extrication.  Engine 1 responded also.  Rescue crew extricated 1 patient.  Engine 1 set up a LZ for the helicopter at Howard Road.

Box 4-B. Dwelling fire.  28 Chan Drive, Washington Twp.

Engine 1 was due in place of DuBois engine.  Station 4 units arrived reporting nothing showing and held engine 1 in quarters before it got out.

Box 4-B.  2nd alarm structure fire.  2908 Airport Road, Washington Twp.

In route: 20:11 (4min), on scene: 20:28 (17min), available: 23:30.

Alerted on 2nd alarm for tanker to the scene and engine to transfer to station 4. Tanker didn't drop any water on scene.  The engine stood by until station 4 equipment returned to quarters.

Box 1-B.  Vehicle crash.  Clayplant Road at Strishock Road, Snyder Twp.

In Route: 18:08 (5min), On scene: 18:10 (2min), Available: 18:45.

Vehicle into the guide rails.  The patient was self-extricated.  Crews controlled the hazards.

Box 1-C.  Vehicle  rollover.  305 Calhoun Road, Snyder Twp.

In route: 15:31 (10min), On scene: 15:37 (6min), Available: 16:03.

Vehicle rollover. Patient was self extricated. Crew provided hazard control. This ended  up being just into Washington Twp. Command advised county to advise Station 4 chief.

Mutual aid. Building fire. 5 S Jared Street, DuBois City.

In route: 07:18 (7min), On scene: 07:32 (14min), Available: 14:07.

Requested for truck to the scene for a building fire. Truck 1 provided master stream on the B side of the building until the fire was knocked down.  Truck then provided access to the 3rd floor B side for mop up and to check for victims. Truck was on scene for over 5 hours.

Crews checked the building with nothing found.

Requested Truck 1 for mutual aid at a building fire.  Truck provided master stream application.

Box 1-C.  Vehicle crash.  7781 Route 219, Snyder Twp.

Dispatched to a reported vehicle off the roadway.  It turned out to be a medical emergency and no crash.  EMS arrived and cancelled station 1 units.

Box 1-A.  Dwelling fire.  1568 Bond Street, Brockway Borough.

In route: 23:43 (7min), On scene: 23:45 (2min), Available: 23:59.

Dispatched to a report of a fire behind the refrigerator in the residence.  Chief 1 arrived and reported a smoke condition in the residence however no fire visible. He held the box to station 1 units.  Truck crew investigated and determined the motor on the fridge had burnt.  They checked for extension and cleared.

Chief 7 requested the rescue for clean-up as Company 7 was late in crewing.  Crews preformed hazard control.

In route: 20:00 (5min), On scene: 20:01, Available: 20:34.

Requested by Brockway Ambulance for a landing zone for a medical helicopter.  Crews secured the football field for the helicopter.

Units: Engine 1, Rescue 1.  Assistant 1-2 LZ officer.

Box 4-C.  Dwelling fire. 6147 Route 1830, Washington Twp.

In route: 11:19 (6min), Available: 11:33.

Reported as a working fire in the attic of a dwelling.  Rescue 6 arrived reporting nothing showing.  After investigating all units were cancelled.  Brockway units were about 3 min. from the scene when cancelled.

Box 4-UN.  Vehicle crash.  Route 219 near the Honda shop, Washington Twp.

Reported as a vehicle deer collision with fluids leaking.  Lieutenant 4-5 arrived on scene holding it to Attack 4.

Box 1-A.  Dwelling fire.  725 Maple street, Brockway Borough.

In route: 11:15 (6min), On scene: 11:16, Available: 11:22.

Reported as an appliance fire in a dwelling.  PD arrived reporting the fire out however smoke in the building.  Truck 1 crew checked for extension and vacated the smoke with natural ventilation.  Engine 1 stood by at the hydrant.  Deputy 1 held it to station 1 units.

Units: Truck 1, Engine 1, Rescue 1.  Deputy 1 had the command.

10-25-2009  00:54 hrs.

Box 1-D.  Vehicle crash.  Route 219 at Steeltown drive, Snyder Twp.

In route: 01:02 (8min), On scene: 01:04 (2min), Available: 03:00.

Reported as a two vehicle accident with one vehicle into the structure with a smell of natural gas.  Chief 1 arrived reporting nobody in either vehicles and no one around the scene.  Also, that the gas meter to the building had been sheared off with a live gas leak.  He requested national fuel to the scene.  Rescue crew controlled the hazards with the vehicles and began a search for possible victims with the TIC.  Engine crew stood by with a line in case of a fire.  Crews remained on scene until national fuel controlled the leak.

In route: 15:18 (6min), On scene: 15:31 (13min), Available: 16:13.

Reported working structure fire.  A shed. Chief 15 arrived reporting a 20x40 wood shed fully involved.  Engine 15 arrived first pulling a line to side B.  Engine 4 pulled a line to side C.  Tanker 1 established a supply to Engine 4.  Tanker 15 supplied engine 15.  Rescue 6 set-up a fill site. All crews worked well together and assisted with extinguishment and overhaul with foam.

Units: Engine 4, Tanker 4, Engine 1, Tanker 1, Rescue 1, Rescue 6, Engine 15, Tanker 15.  Chief 15 established command and turned it to asst. 4-1.

Fire alarm at the Laurel Mountain Winery.  Units cancelled while in route.

Box 1-C.  Vehicle crash.  7759 Route 219, Snyder Twp.

In route: 15:28 (6min), On scene: 15:31 (3min), Available: 17:47.

Dispatched for a two vehicle crash with injuries and entrapment.  Chief 1 arrived reporting entrapment in both vehicles.  Station 4 was requested to assist.  The accident involved a mini-van with 4 occupants, one entrapped and a car with one entrapped. Rescue 1 crew extricated the patient from the mini-van. Engine 1 crew pulled a protection line. Attack 4 assisted with removal of the victim from the car. Engine 4 set up the LZ for two helicopters.

Rescue crew controlled the hazards and assisted EMS.  Engine crew diverted to the football field for a possible landing zone.
